Shortness of breath on exertion

In January of X year, a patient became aware of breathlessness on exertion and was diagnosed with heart failure by a general practitioner (family doctor). She was treated as an outpatient, but her condition did not improve, and was transferred to a general hospital. After careful examination, the patient was found to have severe aortic stenosis (AS) and low cardiac function due to complete left bundle branch block, and was referred to the department of cardiology at Aomori Prefectural Central Hospital in March of X year for further examination and treatment. In late May of X year, the patient experienced worsening shortness of breath on exertion, and despite diuretic adjustment, there was no improvement.
